The four package tiers: base, extended, executive, exclusive – which for whom?

Although every insurer names its packages differently, the market organises around four typical tiers. Understand these four, and you can place any offer on the map.

Tier 1: base – the outpatient core

Specialist consultations, labs, basic diagnostics – no waiting lists, phone-organised. This tier delivers the „no more waiting weeks for a specialist" promise and typically includes basic screening. Most companies start here for the wider team.

Tier 2: extended – diagnostics and screening

Adds imaging (MRI, CT), a richer annual screening programme, often physiotherapy and add-ons. This tier carries colleagues through more serious diagnostic journeys – the most common choice for middle management.

Tier 3: executive – inpatient and comfort

Inpatient and surgical covers, executive-checkup-depth annual protocols, second medical opinion, higher limits, and often reimbursement-based freedom of doctor choice. A retention tool for key people – and a highly communicable element of the compensation package.

Tier 4: exclusive – the extended circle

The executive tier extended to family members and premium services. Contents here are highly bespoke – typically tailored for owners and top management.

A well-designed programme is a mix

The best structures are not one-size-fits-all: tiers can be set within the company (staff–middle management–executive), fixed with objective criteria. The budget goes where it is worth most. Exact tier contents differ by insurer – which is precisely what our comparison table lines up. More on pricing in the cost article and on the Packages page.
